VA Forms
Veterans/Selected Reserve
So that we may assist you with your VA Benefits, please bring the following information to our office located in State Hall.
- Completed Application for Veterans Benefits:VA Form 22-1990
- A copy of your DD-214, if discharged from active duty.
- A copy of your NOBE (Form DD-2384-1), if you are a member of the reserve forces.
To use the Web Automated Verification of Enrollment (WAVE), you must have a current or active benefit award for MGIB-Active Duty (Chapter 30) or MGIB Selected Reserve in the system. For automated telephone certification, call 1-877-823-2378. To only change your address, call 1-877-838-2778.
